Only children who will be four (4) years old by September 30, 2008 will be allowed to register. It is not necessary for children to be present for an adult to register them.

On Registration Day, parents/guardians are asked to bring the following items:

• Proof of Residence for your school district (Utility Bill, payroll check stub, rent/house payment receipt, etc.)
• Proof of Income (current income tax return or food stamp letter verifying names and numbers)
• Certified State Birth Certificate (not a hospital copy or mother's copy)
• Social Security Card for your child
• Proof of a current physical exam and dental checkup
• Green TN Child Health Record Card - This card can be obtained from the Health Dept. or your Health Care Provider and must be completed with all required immunizations. The Health Examination section of the TN Green Card must be signed by your medical provider and proof of dental exam must be provided with a statement of dental health.
  Note: If your child has an incomplete shot record, he/she must have a Valid Temporary Record

If the above items are not available at the time of registration, please register your child anyway. However, these items need to be secured as soon as possible and will be required in order for your child to enter Pre-School.
